Positions available:

Night On Call- Position Description RN- FT

RESPONSIBILITIES:

1. Triage telephone calls and respond to patient/family calls in a timely manner, focusing on
individual patient needs.
2. Perform emergent patient visits in the field (i.e. admissions, pronouncements), as needed.
3. Teach, supervise, and counsel the patient and family regarding care and other issues related to
the patient’s illness, including pain and symptom management.
4. Provide anticipatory guidance to patients/families to assist them in preparing for and coping
with the events as they occur throughout the disease process.
5. Implement the nursing portion of the plan of care in coordination with the other members of the interdisciplinary team.

QUALIFICATIONS:
1. Ability to relate well to terminally ill patients and their families.
2. Ability to assess patient/family needs and to be sensitive to those needs.
3. Willingness to function as a member of the interdisciplinary team.
4. Be familiar with the hospice concept and related nursing issues.
5. Experience with Electronic Medical Records a plus

PROFESSIONAL REQUIRMENTS:
1. A registered professional nurse from an accredited nursing program. Currently licensed to practice nursing in New York State.
2. 2+ years of RN experience (Hospice, Med-Surg, Homecare experience) required.
3. Experience with Electronic Medical Records a plus.

SCHEDULE:
Two 12-hour weekend shifts, 9pm-9am, within a biweekly period.

Five 12-hour weekday shifts, 9pm-9am, within a biweekly period.
