Cathlin Brown Wolanske, a certified Five Wishes presenter, along with Mary C. Crosby, President and CEO of East End Hospice, will introduce Five Wishes, one of the nation’s most widely used advance care planning tools. Written in clear, everyday language, the document helps individuals express their medical, personal, and spiritual wishes for future healthcare.
Attendees will learn how the form works, have an opportunity to ask questions, and receive a free copy of the Five Wishes document to complete at their own pace.
